RELEASE WAIVER (Waiver must be agreed to receive a competition number)In consideration of my entry in this exercise, I, the undersigned, intending to be legally bound, do hereby for myself, executors, and administrators, waive and release any and all rights and claims for damages which I may have against any community and location where a sanctioned event is held, the Canadian Disc Dog Association, their representatives, successors, employees, assigns, and/or sponsors for any and all injuries, or illnesses suffered by me or my dog as a result of my participation in said event. I attest and verify that my dog is physically fit for this exercise, and hereby assume the risk of any canine disease that may be contracted in said event. By my signature I also grant full permission to the Canadian Disc Dog Association and their assigns to use my photographs, videotapes, motion pictures, recordings, or any record of any sanctioned event for any purpose whatsoever.
